I see most of this discussion framed as either or, but truthfully, I almost always use both at the same time. That, however, gets into the muddy question of linking, which is not likely the majority of cases when people are using Afflictions. Well why not? Even if it just me softening up a villain ...

Dazed from damage and Dazed from an Affliction aren't quite the same thing; the former goes away after one round, while the latter doesn't (and since saves no longer degrade, depending on the save involved, this can last quite a while).

The other way to look at Fiat is, is it a bad idea to let the villain get defeated by an Affliction? Would you do the same thing is a player hit with a Power Attack and a crit and the villain rolled a low number which would result in them getting knocked out? And does it make a difference if it hap...

I think the various things that create it are mired enough in a combination of game mechanics, structural features, and differences of perception between players and GMs that there's no easy address of it. The whole "aren't allowed to work" issue however is entirely a matter of GM preference though. Your argument that GM fiat is used every time a good affliction lands only applies if the GM does in fact use fiat every time. As someone who uses Affliction-type powers in 2e I'd say they get used less because the cost more. Well, that didn't help but a lot of people would set up an array to hold a 3/rank power anyway (for areas or whatever) so a ranged Affliction wasn't a painful thing to toss in there.
